Social Care Nurse Educator
Jan joined Partners in Care in 2022 as Social Care Nurse Educator to give advice and support where needed. She believes Health and Social Care integration is the way forward providing optimum care and support for all patients/clients.
Jan has been a Registered Nurse in the NHS for over 25 years. She trained in Wales and worked for Shropshire Community Health NHS Trust for over 20 years. She has been a community matron in Shropshire for the last 10 years, caring for patients with long term conditions and has supported patients and carers in their home environment and various care settings within the community.
She has a BSc (Hons) in Community Health Studies and a Diploma in Nursing, and has been an independent prescriber for over ten years.
